HeyClaire, I am pretty sure waviers are only for active orthodontic treatement, but if you are worried, I would contact the Academy - the staff othodontist's phone number is on pg 6 section 13d of the PTR instruction booklet. Good Luck.

The orthodontic waiver is only for people who currently have something glued to their teeth (hence, the need for a wavier). If you had braces, but they are off now, you do not need a wavier. If he never had braces, he definitely does not need this form :)
