DoDMERB Re-Applicant Question

Hello,

I know this is a common topic so I apologize if this question has already been answered at some point before. I am a second time applicant who was medically cleared by DoDMERB last year when I applied. I understand that I am only required to sign a current-health form to satisfy the USNA's medical qualification, but I have a couple of questions.

First, is there any action required on my behalf besides getting my USNA application to 50% completion (which has been done for some time now)? Such as, do I have to contact someone from DoDMERB / log in somewhere, or will the information simply be emailed / mailed to me?

Second, I understand that DoDMERB is 10-15 days behind because of the holiday season, but when they eventually (hopefully) contact me regarding this current-health form, will it be via email or physical mail? I only ask this because my home address on my USNA application is in a different state than where I am currently attending school.

If I remember correctly, they mailed the health form last year. It was a simple page. The Dodmerb clearing just goes straight to your portal, whenever they decide to update it. Hopefully someone who has done it this year will chime in. Good luck!
 
Dodmerb Website showed "remedial requested". Got letter in mail with instructions to print off a 3 question form to fill out and fax/email back to them to let them know about the present health condition and changes since last exam.
 
I've had the same problem too about the "current health form". I haven't received an email with the form but sent it anyway in the mail. I have everything else done but if this form is not received/confirmed by the deadline will my application be withdrawn from consideration?
