PLEASE HELP, I am filling out the medical history section for DODMERB and one of the questions asked if I have ever had exercise induced asthma. I have only ever had one attack when I was 12 and I don't think I really had it. I was given an inhaler and prescribed medicine but it was really just because I was out of shape that I had a wheezing attack.

so my question is if I will be medically disqualified or if I will have to get a medical wavier?

heres my two cents...
a lot of the disqualifications occur due to a history of some symptom or condition
your case doesnt sound like its a history, more of a single incident if i understand you correctly.
what i would do is put down the occasion and incident, and explain it the way you did just now in the remarks section they give you. this way, they can deal with how to judge the case and you wont be held up for hiding something from them if you didnt report it. you probably will not be disqualified, but you might be looking at a waiver for something they mark you on. the key is to be totally honest but dont diagnose yourself with any unnecessary things.
just for your heads up, i am a candidate for all 3 academies and i have gone through the DODMERB process so i am giving you the most informed opinion that i can.

Larry Mullen rocks! He helped my daughter navigate through an asthma DQ and into an eventual waiver. We also met him at a service academy day for our MOC and he's a genuinely nice man.
 
Also to answer the OP, the formal DQ is written as follows:

D241.30 Asthma/reactive airway disease/exercise induced bronchospasm after age 13

Don't quote me, but that you had an incident at age 12 and nothing afterwards makes me think you'll not receive a DQ for asthma, but Larry Mullen is the expert!
