The classmate of mine just contacted me about this, as he was the O4 commander whose career is destroyed by the GOMOR he received for this.

Sounds like some creative scapegoating.

Someone approved the mission or it was rogue.

He was indeed out of the country at the time.
 
Sounds like the typical stuff they try and hang a junior officer for..."failure to adequately train or prepare...etc...." From all accounts I have seen, the men under his command fought with courage and bravery and as one would expect from SF soldiers, they were also extremely resourceful in a horrible situation. Seems to me he trained them well and should be given a medal.
