Waiver approved but no LOA

If your DD already has a nomination and the waiver, odds are good that an Offer of Appointment will be coming.
If she received the waiver but has no nomination, it is up to USNA as to whether or not they go actively looking to fill that nomination (with a Superintendent or VP nomination, for instance).
There is still hope.
What if the waiver came very early in the process, like in early December, but there’s no appointment yet?

It just means you get to wait longer. I've seen waiver granted to appointment offered time frames of 4-6 months! (especially if you finish everything early) The wait is brutal. Wish I could make it better, but at least you finished early and you're not running around getting medical records in March.
