Permit to Report Package Questions

Hello! I'm currently filling out the permit to report package and I have a lot of questions. There are a bunch of terms I'm unfamiliar with so I made this thread to ask for some help. Thanks!

To start off with:
Base Access Pass Registration:
-What is a Base Sponsor Name
 
Medical Record:
I went to my doctor and they said that they could not fill out the Section under Immunization Staff Use only. Is that fine?
 
Block #25 "Base Sponsor's Name would be your Midshipman's name; an Alpha number if you know what it is but not necessary." (Copied right from "How to complete form 5512.1") These instructions are included in the PTR Package.

"For Immunization Staff Use At NHC Annapolis Only" I think this is the section to which you are referring. I would say leave it blank

I believe immunization staff is referring to staff in Annapolis on I-Day. The top/middle part of the form is for you and your doctor.
 
Is there any way to know if the Academy received your PTR packet? My DS mailed his last week but didn't send it priority or with tracking. Fortunately we have 3 copies of the entire packet of everything sent. I will bring a complete set to I-Day, but would like to know if everything was received before then.
