Immunizations that happen after yellow form sent?

DS will be getting one immunization after the yellow form will already have been sent. I know it says "Bring two copies of this form to I-Day along with any updates." It sounds like he can hand-carry proof that he received the immunization, but does anyone know for sure? And should it be a letter from the doctor, or should a whole new form be done, or what?
 
my DS had that same issue. he decided to send the yellow for a few days late, after all the immunizations were complete and it worked out fine. i think he called to check on that, and they said 'yes, no problem if it's a few days late'

you can also make copies of the form, and add the new records to it before showing up

So, your estimation is that he is deficient by 1 immunization, after his doctor office completed the form?

Why not submit the Form with "what has been done" .... then let the Navy Medical Staff decide what needs to be done to bring him up to standard?

We called NAPS (same form as USNA) and told them that it looked like DD needed 2 different updates to her Immunization schedule .... They said send the form as is, and they will deal with the rest.

The yellow form itself says to bring any additional updated info with the 2 copies of the info you already submitted. You will be fine to do the additional immunization and take documentation.

On a side note...the Bexsero (required meningitis B shot) is a series of 2 intermuscular shots 1 month apart. Over 80% of people report moderate pain at injection site with alot reporting severe, as well as 30-50% reporting headache, fatigue, and nausea.

DD has never had much reaction to immunizations but felt like she had the flu for a day or two after Bexsero. I am very happy she will be done with her second dose before I-day.

Just something to consider when deciding to wait.
 
Exactly, USNA77, the reason he will be one immunization short is that we are setting up the Bexsero prior to I-Day, but it won't be completed by the yellow form's due date. I also didn't want him getting it during plebe summer, since he will already be getting the Adenovirus pills, and since they are live viruses, I thought there would be a better chance of him having side effects if the Bexsero was thrown into the mix.
 
Our family practice did not administer Bexsero when DS went in last week. Not great to hear about the side effects. I think we need to look into getting it (at least dose 1) in the next couple weeks. Thanks for posting this.
 
You should send in the yellow immunization form by the due date. Just have your doc update the second (out of 2) bexsero doses on the two copies and bring to I-Day. I mail my yellow form, PPD test result (also included on yellow form), and the patient registration this morning. Will be getting second dose of Bexsero early June

PS: I had side effects from the first dose, but nothing too crippling
 
Just an FYI, if you go on the Glaxo Smith Kline website, they list where Bexsero can be found. We are getting ours at a pharmacy-in-a-supermarket. Most of these in-house clinics/pharmacies can get the drug and administer it.
 
My DD got her bexsero shots at the supermarket pharmacy too which were covered by our insurance. They will fill in the navy yellow sheet if you still have it. If not they will print out all the documentation you need. I had them fill in the information on a copy of the yellow sheet and attached their printed documentation. My daughter brought it to Iday and it was fine. Get it before you go if you can. My daughter did not have any side effects.

Thanks for this heads-up. DS was able to get his first shot of Bexsero at our local CVS walk-in clinic. After the shot he dropped and did 20 to move it around from the shot loci and for about 2 hours announced “What’s the deal?” An hour later he said it was the most painful shot he ever had. Good thing it was in his left arm because he has to pitch this afternoon.
