If a cadet applies for USUSHS, they essentially will attend the university upon commissioning with no charge, etc in exchange for a 7-year active duty service commitment, correct?

Does this add upon the initial 4-years after commissioning which means 11 years total after graduation from USUSHS or are you considered A/D and the time you spent in USUSHS counts towards the service commitment?

Edit: I'd assume they'd just accrue it so 11 years total because of how much resources they spend on you but just wanted to verify.

Your final 'payback' really doesn't begin until after completion of residency. So 11 years (specialty dependent) would be after completion of residency not medical school.
