I initially filled out the orthodontist form for DODMERB, and got that signed and sent off. USNA reached out to me on this, it was completely random.

Thats what I thought at first but with looking at the forms its all information about my current treatment as well as a last page stating that If i sign this i am agreeing for another orthodontist to complete my care.
