My DS read this completely differently. He thinks it is on everyone’s portal and does not even pertain to SLE. His understanding is that it is directed to candidates who have been admitted to the USMA class of 2022.

My DS had the same problem and used the “contact us” at the bottom of the medical questionnaire page and let them know. Today he received an email with the needed PDF with the medical questionnaire. Hope this helps!
