When I filled the information out I just picked a random date towards the end of May because that is the class of 2022's graduation date. Try May 2026.

How do we combine medical records if they are from multiple different sources? I have vaccines from my doctor, CVS, and Walgreens and you can only submit 2 documents. Should I scan them all into one combined pdf?
How should I report my successful surgery? Should I try to find files to send in? DoDMERB emailed me back and told me that there isn't any paperwork to fill out and just to notify them if I have any adverse reaction or condition.
For me, it just said to list at least one relationship. Should I fill one out for each parent if their contact info was already filled out under emergency contacts?
