2022 Athletic Questionnaire

My son is looking to attend West Point and is hoping to be looked as a basketball recruit as there as well. Most schools have a sport recruitment questionnaire, but the link for the USMA questionnaire no longer seems valid. Anyone know of an updated men's basketball recruitment questionnaire?

Odd that Basketball seems to be the only link that doesn't work. I suggest emailing the coach directly, similar to instructions for football and hockey.
