I am in the process of filling out my NROTC scholarship application and I am on the recommendations page. There are blanks for my counselor, my science teacher, and another rec that say "Name, School, Email, etc."...but I see Address? Isn't that too personal or do I not have to fill that out?
 
The address I put was school/organization address. Just ask them if you aren't sure.
 
Always use the official school mailing address.

Regarding email, it is helpful to give your teacher a heads-up to watch their spam folder just in case the military email gets filtered out of their in-box.

Yes to this, and also send them an email explaining what you are doing and that time is a factor. We initially asked DS math teacher for the recommendations in May. Then in September. Then in late November (with a face-to-face request from Mom) Then again in late December. FINALLY she did it two days before the deadline. With each of those requests we had to contact the Academy to resend their request for the recommendation.

It is surprising just how many school teachers are almost completely unfamiliar with the U.S. Service Academies and how prestigious an appointment is.
Fortunately, my DS's school counselor was an Army Brat, so she was VERY familar. For the others, my DS sent a link to the Academy website to clarify what he was seeking to do and like @eljay60 said, had a face to face discussion about it.
