A3425 Form

My MS-1 DS has a list of forms to complete in addition to the ones he did before he went off to college. He needs a A3425... basically a physical. His Dr. is 2600 miles away. Is this something the Dr. who did his DoDmerb exam would sign?
Form is very short.

Have him schedule a physical exam at Student Health. It's free at your son's school (I have a child that went to the same school) with the paid Wellness Fee. Or, if your/his regular medical insurance covers an annual physical it could be done that way at a 'doc in the box'... It's just like a sports or school physical. It's not related to DoDMERB.
