It has a folder with official signing/rejection information, package to mail back, acceptance congratulatory letter, and a cardboard like folding-certificate of your appointment

I believe there is something in the Bear's Den that you have to select to have the package mailed to you.

If you received a conditional appointment do you receive another acceptance letter after medically qualified? My DS doesn't see any enrollment questionnaire with his conditional appointment letter.

Yes. You will get a new letter that says it is a full appointment.

I also had the same question; my son received the conditional acceptance (waiting for medical waiver) and has not received letter in the physical email or any link for enrollment questionnaire.
