I'm currently applying for the summer AIM program - on the Extracurricular Information, I need to put my advisor's name and their phone number. Should I ask for their permission first?

Yes! Not only is it a courtesy, it will also give you an opportunity to educate him/her about service academies. Many people are unfamiliar with SAs and the awesome opportunities and preparation they provide.

It is always a good idea to ask for consent, but it shouldn't be an issue if you don't ask before putting their name down after all they are your advisor.
