All required forms for an official offer of appointment

Hello, I received an LOA some days back and I was just curious if anybody knew about some of the required forms that come with getting an official offer of appointment, like police background checks, etc. I know I still need to hold out for the official appointment, but I'm just a bit curious about everything that you must submit after you accept your appointment. Cheers!
 
My son quickly showed them to me ... police background check, body piercing and tattoo statement, proof of citizenship.

I will see the official stuff later.
 
If it is similar to USMA, my DS was contacted a few days after accepting to get fingerprinted. Then a week later with some admin questions. Then another week later with a huge questionnaire on background check, etc. This one seems to be more general to the DoD. He has been uploading all sorts of files into the portal. They provide deadlines and encourage you to start early. He is taking it one request at a time. Best wishes.
 
Police record check can be a PITA depending on your locality, get on that ASAP once available. [It's discretionary for local Law Enforcement if they will or not.]

May as well organize your immunization records now if you haven't seen the same pediatrician since birth, might take time to request records from old doctor's office etc.

Also if at all possible get fingerprinted at a military recruiting office; they can upload digital prints directly.

Our school has the absolute best police officer ... resource officer. My son printed a copy for him and he said he would take care of it immediately. He was enlisted in Navy before he became a police officer. Everyone loves him.

The forums when you accept your appointment are pretty minimal right now. Usually the body alteration form, ensure you have your birth certificate, and police check. The Permit to Report will be longer and comes out usually in early April and has many more documents. Your SF-86 is your document for a security clearance and is rather lengthy and will come in the PTR.

Our police department would not mail the background check directly to USNA per the directions . DD contacted admissions and they said we could just mail. Admissions seemed to be familiar with this issue. Don’t stress if that is the department policy in your area. Of course follow the directions and have the police department mail directly if they will.

We can’t get any of the local police to even fill it out. We are going to try state police next- frustrating!
 
Wow!!! I am feeling old.... When I did all my Security Clearance BI's, they were done by civilian DoD Investigators, with credentials. And they ran NCIS, NLETS, and local police checks, then actually went and talked to your neighbors from ten years ago, and your HS teachers. Now, I am guessing everything is electronic, fill out a set of questions and have the local police submit them over NLETS and then send the results to the academy. In my day, the results were sent to Headquarters by the investigator. It usually took three months. Now it is probably almost immediate? Technology...almost scares me:)
 
They still do interviews for the security clearance process by trained folks from OPM. That has not changed, but the police background check is not that. Two different things. USNA has the police background check step to ensure somehow an appointee didn’t have something that no one mentioned or was lied about during the application process. The security clearance interviews can occur well after I Day so this is a quick check prior to that.
 
If you want something to do in the meantime, you can start getting ready for the first 3 things they'll ask you for;
1. (Police Record Check) Check into what you need to do at the local or state level police office for a Police Record Check. USNA will give you a one-page form.
2. (USNA Body Alteration Form) A four-page questionnaire on tattoos and body art, to fill out and send to USNA.
3. (Proof of Citizenship) get an original or certified copy of your birth certificate to mail to USNA. They will send this back to you.

If you are under 18 years of age, you may find that the police records check request is different for you. There is often fingerprinting involved in this process, so you may have to have an appointment.
Might as well call now and get the lowdown on the process. It will give you something to do, anyway.
