Allergic rhinitis?

Hi,

I recently completed my DoDMERB physical and was just sent a request for a remedial.

For clarification, I had an ear-tube removed in 2000, when I was in 3rd grade. The doctor noted allergic rhinitis on the papers, and did not recommend treatment or offer any explanation. Since that time I haven't had any problems.

So when I noted allergic rhinitis on my history paper, I basically said that^^ and expected no problem since it was so far back in my medical history. However, DoDMERB now would like paperwork and other things that I don't have. I haven't even seen that doctor since 2000.

I'm not mad, I'm just wondering- what is the appropriate way to handle this? Is the diagnosis paper enough? I really don't have much else.

I would be very appreciative of any help! :smile:
 
Provide what you have and provide an detailed explanation of what you may not have. Also, the fact you haven't seen that Doc since 2000, doesn't mean that he doesn't have the records. Pls provide your Full name and Last 4 of your SSN to Larry.Mullen@dodmerb.tma.osd.mil:thumb:
