As part of my AMI, the dodmerb requested the Orthopedic Questionnaire be filled out by me and my dentist regarding the retainers I was wearing. Since then, however, we have stopped treatment and the buttons have been removed. How should I report this and can I submit my AMI without the questionnaire?
