AMI R251.00 contacting Civ Team

After review of my son initial Medical Examination (IME), Additional Medical Information (AMI) R251.00 was required.

R251.00—Basically states that on DD form 2808 of the IME the examiner did not properly fill out the form. In my son’s situation #34 lower extremities was not checked. Nothing was marked. just left blank.

The directions state that he is to contact the Civ Team and a phone number is given. The phone number ultimately leads to a voicemail box. Our concern is that after multiple messages left to this voicemail no one has responded, and no change to is status has occurred. Has anyone had to deal with the Civ Team? It seems like a very simple fix. He has nothing wrong with his Lower Extremities. Anyone have any idea as to how we can move this process along?

DS had physical in January and the provider did not complete the form properly, leaving 2 boxes unchecked — probably the same area(s). Civ Team was very helpful, but DS had to go back to a provider to get just those items marked off appropriately. Civ team gave the option of going back to the provider that they initially set up or to use our own provider if that was easier. It was resolved very quickly.
