My son was asked to submit an AMI. We were uncertain who to list on thr form, as there were 4 lines to fill out, with name, DOB and relationship. He put me, his mother. Whom does someone normally list on an AMI? TYIA!
 
Just bumping this question up.
Typically, Mr. Mullen, Deputy Chief, DoDMERB, kindly volunteers time here and answers questions, usually on his own time, after hours, and often very late. He may be on travel or PTO, but he will usually respond at some point.

If your son has questions, he can always contact his assigned DoDMERB technician. Go to the DoDMERB website, look on the left-side menu, select Contacts. There is a list of technicians associated with various chunks of the alphabet.

If your son hasn’t yet read every item in that menu, it’s very helpful.

 
Thank you for responding. We read every word on that website it had all the info I was looking for. Except... this AMI form. Who do people usually put? It doesn't seem like a doctor, since they ask for DOB and relationship. Is it bad if you only put 1 person? The AMI form only asks who he authorizes to talk about his medical history and I'm really the only one who knows anything.

Thank you Director Mullen, we were able to clear the remedial code that prompted the AMI request. We are now in UNDER WAIVER REVIEW status. I will have my Cadet reach out with any questions on that process.

Subquestion from me: His DoDMERB for 3 year AD scholarship activation was initially clear a year ago with only one R code for invisalign. A year later he sent in the documentation to remediate and activate the scholarship and suddenly 2 DQ codes came in. Is this normal?

Your son, the applicant, should have the subquestions...I wouldn't know what was normal, until he contacts me and I can look at his file :wiggle:
