An Inconsistency With Medical Records

I was looking through my medical health summary, and I realized there is some irregularities with the records.

1) I remember getting a very minor fracture in my skull when I was around 5 years old and getting stitches for the injury. I got the treatment at the same hospital that I am currently with.

2) I have a tree nut allergy (completely fine with peanuts) and a shellfish allergy, both of which are not listed in my medical records.

I find the first case to be quite odd, as it seems like a thing that I wold have a record for, but as far as I know, I don't have one. For the second case, though I have those allergies, which are VERY VERY mild, I have never been properly diagnosed with having any allergic reaction to any of those substances.

When I do my DoDMERB, I plan on disclosing this information to the physician, but given that there are no medical records for these given conditions, should I even report them? As far as I know, those records do not exist. What further actions do I need to take if I were to disclose this information? Are there any waivers I should start on? Also, are any of these conditions disqualifying for AROTC?
