Asthma/RAD questionnaire R252.13

DS had his DoDMERB exam back in January. When he checked his status middle of last month it showed a code of R252.13 and showed they mailed him a questionnaire for asthma, reactive airway, exercise induced bronchospasm on Feb 10. He still has not received anything in the mail. so he emailed them and was told he could download the form and send it back by snail mail, email or fax. The code said no doctor visit was needed. I'm just curious if anyone has had this same code and if there was anything else in what was mailed that needed to be addressed. I just want to get this taken care of right from the beginning so there are no hitches down the line.

When he was 3 years old, DS had some wheezing during the winter and went to the ER. Doc there called it RAD and gave him an inhaler, which he never used. Never had an issue again and played multiple sports year round all his life. The kid has no breathing issues. I was a bit surprised to even get this since it was when he was 3 and no further issues.
 
IMHO I think this is good news. I feel that DoDMERB is just doing their job and following up on the reported Asthma, gathering a little more information so they can either qualify or forward for a waiver. Below I have copied the standard for Asthma. Look at the criteria and it may guide you on how you word your answers on the questionnaire. I bolded what I consider key points. Remember this is just my opinion, no ties to DoDMERB or the process, and on an anonymous forum.

d. Airway hyper responsiveness including asthma (493.xx), reactive airway disease, exercise-induced bronchospasm (519.11) or asthmatic bronchitis (493.90), reliably diagnosed and symptomatic after the 13th birthday.
(1) Reliable diagnostic criteria may include any of the following elements: substantiated history of cough, wheeze, chest tightness, and/or dyspnea which persists or recurs over a prolonged period of time, generally more than 12 months.
(2) Individuals DO MEET the standard if within the past 3 years they meet ALL of the criteria in subparagraphs 11.d.(2)(a)-(d).
(a) No use of controller or rescue medications (including, but not limited to inhaled corticosteroids, leukotriene receptor antagonists, or short-acting beta agonists).
(b) No exacerbations requiring acute medical treatment.
(c) No use of oral steroids.
(d) A current normal spirometry (within the past 90 days), performed in accordance with American Thoracic Society (ATS) guidelines and as defined by current National Heart, Lung, and Blood Institute (NHLBI) standards.

DODMERB does not know the nature of the RAD, or if there were further issues, hence the questionnaire. The medical history form he filled out just showed that he had an issue in the past.
 
@5Day @MohawkArmyROTC Thanks for the info. I thought there was some follow-up questions on the medical history that addressed when this occurred and if there were any additional issues, but I could be mistaken. I know his answers to the head injury questions prompted him to fill out the head injury questionnaire before he even had his exam (concussion as a freshman). I realize DoDMERB is just doing their job and I am not worried about him being cleared once they have the info. I just want to be sure we file the right paperwork the first time since we did not get the mailed information.
