Authority to Release Medical Records Form

Has anybody filled out this form yet? It was emailed in a packet from the cadet medical clinic. We can't figure out the form.
 
My son looked at this tonight. He filled in his name, the person (mom) who he is authorizing to get medical info if necessary, and left the rest blank for now. I would think this form would have to be signed in front of a witness (at least at our doctor's office we always have to do it in front of someone), so he didn't sign yet. He thinks the date not to exceed one year that it terminates will be a year from in-processing, but didn't want to assume so left it blank. It said in the cover letter that it should be hand carried to the AFA and turned in on in-processing day so that's what he'll do. Hopefully they won't give him a hard time for not filling every blank--it should be a quick thing to do that day.
 
does this get emailed to you after you submit the medical forms in the appointee booklet? ds hasn't gotten anything like this so just wondering if we missed it somewhere in the packet?
 
My son got an e-mail on May 17 with the subject line USAFA Welcome Packet for Class of 2015. There were 7 attachments--a welcome letter, something about the HPV vaccine, 3 airmanship documents (medical stuff), eyeglass form, and the medical release form. The eyeglass/contact form (if you wear them) is the only one that has to be e-mailed or faxed back in by June 1. The others are to be hand carried on in-processing day. Hope this helps!
