Hi all,

I just had a quick question about the body alteration form that you have to send in after you accept the appointment. It asks for a witness AND the applicant’s signature, so I was wondering who this “witness signature” is for. Doctor, family member, specific third party? Thanks for the help! I really appreciate it.
 
It can be anyone over 18. Neighbor, father, as long as it doesn't say exactly who it has to be.
 
I was also wondering but I went to my Doctor and they told me that they couldn’t provide a signature as a witness because their name can carry weight. The Doctor just said anyone could sign it so my DM did and we sent that in. I’ll let you know if they accept it.
 
My Dad did it and then we got it notarized and sent it. It has been accepted and is marked as received on the portal.
