Can you fix mistake in initial report?

Hi there.
My daughter had an issue with headaches in 2017/2018.
She saw a doctor and took medication until 4/18.
When she filled out her DoDMERB intake report she reported daily/weekly headaches that prevent you from taking test effectively. She meant that when she was under doctors care that is what her symptoms were, but she has been asymptomatic since 4/18.

She filled out the form incorrectly.
Can she change the original form?

She is being DQ'd because of it.

And.... your daughter, the applicant, has taken what steps with a DoDMERB to correct the mistake she made?

Hello, first post here (long time lurker). I didn’t start a new thread as my question is the same as the OP.
And I’m a little stunned: we were told it wasn’t possible to correct any answers entered on the initial medical questionnaire.
In his first SA application we assisted my then-17yo as he completed the form, taking the warnings to be honest very seriously, of course. We didn’t encounter the sage advice not to “diagnose yourself” until we discovered this invaluable forum and after we’d already gone and done just that.
Can anybody tell me, is it really possible to correct those mistakes?
(DS is now a re-applicant with an LOA and could receive a waiver this time. But he had an LOA that first year and did not receive a waiver so he’s understandably nervous.)

Yes....kind of. The system is designed that way on purpose.
An applicant can submit a statement to DoDMERB as to WHY they put the answer they did and then WHAT they submit their REAL answer should have been.
DoDMERB will “consider” the statement.

DS is currently ‘under DODMERB review’, should a statement be emailed to his tech?
