What is in the USNA Candidate Academic Information Application and what high school official should I designate to fill it out?

It includes all sport, student leadership. and awards. Your guidance counselor is the one to verify.

2018midmom, I think you might be confusing the Candidate Activities Record with the Candidate Academic Information Form. As stated, your guidance counselor is probably best suited to fill this out, though a school's registrar could also provide the input, if necessary.

On the activities report it asks for guidance counselor. The academic report only asks for a school official
