Can't Access Appointee Kit Forms

I don't have access to the computer I downloaded the O-143 and SGLI forms on and I can't open them on the appointee kit since I already submitted it. Does anyone have the forms or a link to them? Thanks!
 

SLGI Form and O-143 Record of Emergency Data

Can someone send me a link to download these 2 files? I thought I had saved them prior to submitting my appointee kit but I can't find them and I can't download them again now that I have already submitted the appointee kit. I have been able to find some with a google search but I am not sure if...

SGLI - Servicemembers Group Life Insurance: This form allows you to sign up for SGLI which you become eligible for as a cadet.
O-143 - This form asks you to fill in information on where to release information to in the event of an emergency & it asks you to designate a PADD (Person Authorized to Direct Disposition) or the person who determines what will be done with your remains in the most tragic of circumstances.

Per instructions in the Candidate Kit, I believe both forms are mandatory to fill out and bring to I-Day.

Do you still need the forms? I download them to my computer before filing them out.
 
Do we need these for I Day 2022? I don't see any reference to them in the Appointee Kit
