Can't remember medical records

What do we do if we can't remember specifics of my dds medical records to fill out the questionnaire? I know she has had some of the things they ask about, but she has never needed continued treatment for anything, so I don't remember. We've requested her records, but she's a military dependent and it is taking super long to get them. I've called Tricare, checked the Tricare online and the new Genesis portals and they don't have anything in them. We scheduled an appointment with her pcm to see if they will let us just look at what they have, or at least check for the big things.

If we can't get anything solid from them, should we just guess and make a note of it somehow? I don't want it to look like she is being dishonest.

I recommend going back and looking at your bank statements and your Healthcare EOBs. That was the only way I could put together a timeline. Good luck+

Did you request her records through your MDG medical records office? I realize every facility is different but maybe talking to their staff will help expedite the process. Good luck!
 
We called around and were able to cobble together a solution. It took driving around all day, but it worked! Making an appt to review her records with her doctor was also helpful.
 
Dealing with this same problem just to try to find a more comprehensive list of medications for the initial questionnaire. We've been Tricare Prime for DS's entire life but moved overseas and AF clinics computers couldn't read Navy records etc etc. His entire medical record is a hodge podge of everywhere we've ever lived with gaps of time because sometimes you couldn't carry a printed copy of your own record, sometimes you could. Sometimes I kept the little paper the pediatrician wrote on, sometimes we never got one. Now with Tricare's Patient Portal you can't access anything once the patient turns 12 and they can't have their own account until they are 18. I don't know what antibiotic he had for an ear infection when he was 6. So really, how are people doing this?

I had to go to a military clinic in person to request records. They'll send the whole file over email. I was stuck in the same age gap you mention.
