Certificate of Acceptance

Hello,

On the USNA Body Alteration Form, it states that each appointee shall attach and submit the Certificate of Acceptance of their offer of appointment along with a photograph of any body alteration.

Where can this Certificate of Acceptance be found?
 
When you accepted online, that was your de facto acceptance of the Offer of Appointment.
If you look at the form, it is a PDF of a scanned image. It's recycled, but still in use.
When you fill it out and send it in to USNA, it is simply updated with the received date in the Candidate Information System.
I wouldn't spend a lot of energy looking for a Certificate of Acceptance - as it may be part of the Permit To Report (PTR) letter that you are required to bring with you on I-Day - or it may have been superseded by the online acceptance.

You don't technically "accept your appointment" until you take the Oath of Office, on I-Day.
