Could candidates be assigned to a Prep School because of CFA scores or is it a pass/fail for both prep schools and direct appointment?

You may want to do a little more reading at primary sources such as the SA websites on the military prep schools as well as the civilian prep school scholarship programs such as USNA Foundation Scholarships, USAFA Falcon Scholarships, etc.

The CFA is just one of many qualifying aspects of a successful SA application. While not a huge part of the algorithm used by individual schools to evaluate a candidate’s qualifications, it does tell the SA the candidate has a certain gateway level of fitness, stamina, and strength to handle the SA’s physical requirements. It’s designed so that a person in the application age range, who is already reasonably fit, with some practice, sweat and no procrastination, can ensure they pass it. There is no reason a candidate cannot successfully self-prep for the CFA. If they can’t, it’s for “will or skill” reasons.

Offers of the military prep school or scholarships to civilian prep school partners are made to candidates whom the SA particularly desires but feels could benefit from a year of additional polishing, usually for academic reasons. Candidates are not assigned but made an offer in lieu of direct appointment.
