Changing responses on medical history form

After filling out the medical history form on the DoDMETS site, I realized that I erroneously checked "yes" on two of the questions. I called Concorde and asked if there was a way to change them, and they told me to make the changes in pen once I had the form printed out. Is this an acceptable way to make changes? Alternatively, does anyone know if there is a way to print a non-draft copy of a blank form and fill it out by hand?

I did the same thing for my form. There is no clean copy for you. Follow their instructions and change them in pen.

I would suggest following the instructions that Concorde gave you.
