Can someone who received one of these nominations confirm all the components needed?

USAFA requires a word doc form, VA Disability Letter and DD214
USNA requires form, VA Disability Letter and DD214

Did those who received a nomination from this category also attach resume, letters of recommendation, transcripts... etc?

Supply what is requested.

These categories of nominations are not competitive. The candidate supplies required documentation. Either the applicant meets the criteria or not. If they do, the nom shows up in their portal.

Any of those other materials might be requested by members of Congress for their nom application process.
