Mr. Mullen. You may or may not be aware but there are two versions of the covid questionnaire on the DODMerb website. The one linked on the home left-hand menu "COVID-19 info for applicants" does not have the statement requiring a doctor exam/sign-off for a positive test.

The one in the forms section (which appears to be the same one as you linked earlier in this thread) does require a doctor exam/sign-off.

Just thought you should know. Unfortunately, my son used the "Covid-19 info for applicants" and I guess it was rejected and he was sent links to the other one...which does require a doctor exam (even though the letter apparently states it doesn't require an exam, LOL).
 
I am totally aware:) One is for the applicant to answer the questions that all applicants must answer.
The other one is for a physician, for those that have tested positive. DoDMERB, specifically designates to individuals, which one or both that are required to be filed out.

I was not referring to the physician's form :)

I was referring to the menu item on the DoDMerb website for covid info for candidates

Which takes you to instructions that are apparently no longer valid

And links to a candidate questionnaire that doesn't say anything about pg 2 physician's form

That when followed (the instructions) causes DoDMerb to send a letter to the candidate saying they are no longer qualified for accession

And new links to a new candidate questionnaire

And the physician form

So if DoDmerb would just change the link on the DODMerb home page for covid info

To the correct candidate questionnaire and physician's form

Perhaps DODMerb could get the complete information they need the first time

Instead of giving dated instructions and links to only 1 of 2 forms

that cause the candidate trying to do the right thing to have a heart attack

By triggering a letter saying they are no longer qualified for accession.

All it takes is changing the Dodmerb menu link to provide the candidate questionnaire with physician statement and the physican form instead of old instructions and a candidate questionnaire DODMerb does not apparently care about.

I won't get into the "not qualified for accession" letter stating 3 times (yes in one letter) that a physician's appointment is not required when it is CLEARLY required.

Just trying to help those coming behind.

Hello, I have a question regarding COVID-19. The DoDMERB website states that:

“Current DOD guidance stipulates applicants who have been infected with COVID-19 can be qualified (or re- certified) no earlier than 28 days after completing any recommended home isolation. This will be roughly 38 days for most applicants, from the time their symptoms began or they tested positive.”

Does that mean that if I get COVID between now and June 23, I’ll be disqualified from entering USAFA’s class of 2026 even if I don’t have any complications?
 
It depends on a case by case basis. You're NOT DQd for COVID-19, but the manifestations of such. @ this juncture, we'd probably defer to USAFA to make the call in accordance with their protocols. You are required to notify DoDMERB of ANY changes to your medical status :wiggle:
