DD Form 4 (Enlistment/Reenlistment Document)

I am in the National Guard and appointed to the c/o 2024, and I am working on my paperwork for a conditional release/service exemption from my current contract. I need to fill out a DD Form 4, but my unit is unfamiliar with this as they have never dealt with my situation. Additionally, my FFR could not provide me any insight on this form. Has anyone had to fill one out? Thanks in advance.
 
So a Conditional Release is a DD 368, you shouldn’t have to do a DD form 4 (reenlistment) unless you have less than two years on your contract on RDay (as far as I’m tracking... please share if you have newer information). All you need is your BCs signature for that and for Block 4 to say where you’re getting released to.
The other thing I would start working on as well would be an Exemption to Policy Memo for the DOD travel ban if this gets pushed to the right as it needs to be signed by General Officers from both the gaining and receiving units. If you need a copy PM me.
 
Technically he won’t need the ETP because he won’t be on orders from the guard. A little different situation than active duty. Throw all the old rules out on this. Get in touch with the guard LNO at USMA for help.
