DD2807-1 Reporting Change in Medical History

Hey all, first time poster trying to find some answers. I am in my MSIII year in ROTC and we are gearing up to go to Advanced Camp this summer. Part of that is filling out the DD2807-1/2808 series of forms to report our medical history. Well, in my camp physical form I marked I had seen a counselor and had in the past had excessive worry/depression. This was due to the fact that in the Fall semester of this year I entered a really dark place due to some life situations and I became very paranoid, had intrusive thoughts, worry, depression, the usual suspects, but after talking about it with someone I learned that it felt much better to just discuss how I was feeling and so I continued doing that, just with my peers/cadre and not a counselor. Since then I have changed the situation in my life (someone who played a role in it that was verbally abusive towards me constantly) and have not had any problems. I annotated that I only saw a counselor once, and after discussion, no diagnosis was made, no medication was prescribed, and there were no follow on appointments. Is the Army gonna DQ/ am I gonna need a waiver for seeing a counselor?
 
First off, the medical form to be used is the DD Form 2807-2...not the DD Form 2807-1. If your Det or Bn gave that form to you, tell them that Larry Mullen said they are wrong. You should be using the forms on DoDMETS...NOT any forms they provide you.

What you should do I’d answer all questions on the DD Form 2807-2 honestly and comprehensively.

I did, just as I did when I initially contracted, got my clearance, etc. I am just wondering how many headaches marking "yes" to those two questions is going to cause me.
 
Contracted? If you are contracted, then you are doing a commissioning exam AND 2807-1 is the correct form. You will NOT go through DoDMERB :)

Correct, we are doing a commissioning exam and I am wondering how my affirmative responses to question 17 (e) & (f) will negatively effect me.
 
Ahhh. Thought so. Commissioning "may" be a tad different, because they've already made an investment in you.

So, I think you kill two birds with one stone. Be as honest and comprehensive as possible (you can use extra paper for additional space.) -- more details than on this public forum. That should shed a different light on things as opposed to a new applicant off-the street. They can also get some command input, that other applicants for an SA and a scholarship don't have...as in demonstrated performance.
