Dental letter from USMA arrived already opened?

I heard there was a lot of paperwork that would come after you accepted your appointment and the dental one was the one to come first. However, it came opened. If someone who has received the letter already this year, can he/she tell me what they found in it? Inside, I found a one page letter describing the dental checkup process, a return envelope, and a two-sided dental form for my dentist to fill out. I am afraid something may have been lost. Seeing that it was open with a knife/sharp object, my dad thinks a neighbor might have accidentally opened it and put it back in our mailbox. Also, can anyone else who has finished the process tell me what else I should expect to come?

My son received this Dental package twice - once in December shortly after receiving his appointment, and again last week.

The December envelope was sealed properly, while the one that came last week was not sealed. The contents were the same - return envelope, information letter, and form for the dentist to fill out for an examination, and a request for X-rays.

My son has returned his results and form already. So far, nothing shows up on his portal that the form and x-rays have been received.
