Out of curiosity are there any actual dental forms that are needed for those who will be attending WP in June?

Have you not received the dental packet? There is a form for your dentist to fill out and certain X-rays must be sent to West Point. I think my son got his 3-4 weeks after he accepted his appointment but I can't remember exactly.

Yes you need a form filled out and a copy of 2 different types of x-rays, but they have to have been taken within the last x month/year timeframe (sorry form not in front of me at this minute). One of my x-rays were good, the other i had to make an appt to get taken again. So definitely call admissions about it.
