Discrepancies in medical files?

Dear respondent,

I am a highly qualified university student with a respectable GPA (~3.6) at a top-10 university, strong leadership experience, and varsity letters from a D-1 team. In high school, I experienced some mild depression and anxiety and pursued counseling. One counsellor gave that diagnosis and kept detailed notes. When I had temporarily seen another counsellor in the same department, a trainee at the time, her diagnosis was far more severe and her notes frequently cite self harm, suicidal ideation, and potential "psychosis" in very specific detail. When she left, I was switched back to the original counsellor who continued with his original diagnosis and kept detailed notes—i.e., we talked about classmates, parents, schoolwork, etc. I have had absolutely no problems or treatment in college and my paperwork confirms I have had a clean bill of health for three years.

I have no idea what the trainee is talking about; I never engaged in any of these behaviors nor "endorsed" them to that counsellor. I am submitting her file because I believe it is the honest thing to do; I would not want my application to be anything less than completely by-the-book. To address her comments, I have a note from the long-term counselor detailing the long-term timeline and his evaluation over time, which is backed by all his notes. I have written a personal statement explaining what I can, but I have no idea why she wrote what she did and I genuinely do not understand it myself—there are some things I can offer no plausible explanation for. I have also gotten a new consult which I have no doubt will be completely clear or any issues. I have already been working with the recruiter for some time, and I did not have ANY idea she had written these things in my file until I retrieved it for the recruiter. Do I have any chance for a waiver? Should I just withdraw my application instead of wasting more of the recruiter's time? I am trying to apply for NAVY OCS (not for aviation, not for nuclear). I do not come from a military background, but it is my understanding that OCS is a commissioning route just as the service academies and NROTC are, and thus this forum is applicable?
