I received a disqualification and need a doctors report or evaluation for a waiver. Does my doctor write a letter with her results and refer to DODMERB? Should see use specific wording for the findings? My email says to return to my assigned technician at the bottom of this letter. There is nothing at the bottom assigning me to a technician for me to send to? Should I just call dodmerb for all these questions? Thank you

Go to the DoDMERB website. Read all of the menu items on left-hand side, including Contacts. You’ll see where you are assigned.

Send me an email: lawrence.e.mullen.civ@mail.mil; provide complete name and last 4 SSN; provide the text of your Parent's posting above to YOUR email. The subject line of the email should be “ten11#43-SAF = DODMERB AMI .“ Do NOT embed links in your email as I will not be able to view those. :wiggle:

DS experienced the following: one note arrived in the portal and he then received it via email as well. One was hard to read due to formatting and the other was a clear pdf.

When you eventually get the AMI read all the words and make sure the answer references all the asks in the way it was posed.

DS printed out his AMI left it with his physician and made this point emphatically after a couple of perfectly reasonable phrasing decisions by the GP that were not on point. Redoing it with mirror language fixed it immediately.

The military apparently frowns on paraphrasing.
