I just scheduled my physical and eye exams. The receptionist told me there were some forms I had to bring for my physical, yet I don't see any links or documents on the DoDMETS page. Where can I find said documents for the physical, and are there also documents I need to bring for the eye exam as well? Thanks in advance.

They should be on your DoDmet portal. You have to download that file, print it and take for physical exam. Eye exam don’t need that file ( at least this was the case with me).

Ah I found the link. The DoDMETS website didn't let me scroll down on the left hand side to see the other links.

How did you log in to the Dodmets site? I am having trouble with my username (SSN) and password (last name)
