is it a good idea to have my daughters medical records sent to us from all drs she has visited over the years to make sure she fills out the form properly? I am sure I will forget something since I have 3 kids. Or do we just go with what we can remember? Does DODMERB request the medical records of every dr you have seen over the last 16-17 years or just ones you answer yes too. My DD is a junior but we are getting prepared for all of this. Thank you.
 
Be very careful with your answers on the form, especially with several kids and different medical issues. My 2023 DD USAFA cadet replied that she had used an inhaler “several years ago”. She ended up being DisQual due to Asthma and use of an inhaler after age 13. It took us 3 months to straighten it out through her doctor’s notes and a list of medications from her Pharmacist. All needlessly. It turned out she was prescribed an inhaler for a bronchial infection at age 10 1/2, and never used it after that. But we had to prove it with medical records or she would not have been admitted, or would have had to pass a Spirometry Test. So, make sure her answers are true, but accurate rather than guessed (better to review her medical records now). DODMERB does not joke around!
 
IMO, that’s unnecessary. If you get to a question you cannot recall, you can call the office and inquire. I have 4 kiddos, and never felt like I couldn’t adequately answer the survey questions.

Don’t overthink it. And no they do not go back through everyone’s records. They CAN request them but it’s not standard practice.

I agree unless there is something in your child's medical history that could disqualify dependent on dates etc.

I’m assuming that something significant would be recalled by either the candidate or parent reviewing the survey...triggered by the survey question.

I’m saying that I don’t think it’s necessary, as standard practice, to contact alllll the physicians over the years and request alllllll the records. To each their own, but I just don’t see that the intent of the survey is for families to comb through 18 years of medical records. My experience with my 4 different (one was twice) surveys is that it’s not necessary.
 
If you are unsure about dates, do you have access to your EOBs from your insurance company online? I used that when we needed dates for sinus surgery.

Also, do not self diagnose. If your kiddo has pimples, no need to say yes to skin conditions such as acne. Unless diagnosed by a dermatologist, many kids this age have pimples.

She has nothing significant but we have started to think about when she has gone to the dr and for what? For example we went to an urgent care for a spider bite that had gotten infected. Neither of us remember exactly when that was. Assuming visits like that are not significant? Not even sure where that would be listed. She had strep throat, etc. So are those kinds of visit not listed anywhere on the form.
 
They are not looking for every bump bruise and doctor visit that she ever had. They’re looking for highlights of the biggies: concussions, broken bones, surgeries, diagnosed and treated acne, eczema, asthma, sports injuries, anxiety, depression, MRIs, drug and alcohol use.

Not ‘normal’ childhood occasional visits like pink eye, strep, colds, etc.

I would seriously not even worry about this now. As you state, she is healthy, and has nothing significant. Very likely your survey will be “no” to everything.

I’m certain you could find an example online via google if your that curious. If you are trying to recall every single visit, you are approaching this wrong imo. Just answer the survey questions. If they want more info, they will ask. And the secret to all this is starting the application early...and completing enough early to actually trigger this exam. You will have more than ample time to address things if you start the application ASAP in the summer. And as you state, nothing significant.

At this point, no need to even think about this with a child with ‘nothing significant’. If a lurker does have open-ended things...then this could be a good time to make sure those are closed. IE: I had one with a knee injury he recovered from and continued through his season. DODMERB needed more info, including a DR’s ‘release to full activity’. We never went for an actual follow up appointment bc he just had a bad bruise. But that was a loose end. Now you can tie those up!

Ok that all makes perfect sense! She is an athlete so yes she has had injuries but no surgery to fix or repair. I did find the form
Online and we reviewed it with mostly no’s and a couple yes on orthopedics. We already have letters from the dr clearing her bc her school required it. I am sure she will be fine. I am glad I asked about the incidental appointments through her life and thank you for taking the time to reply. I appreciate it. She is super excited about applying!
