DODMERB medical history mistake

I accidentally clicked yes for skin conditions, now I can't change it and it keeps asking me to name physicians, diagnoses, and dates. I can't change it to no. I was told to cross it out on the form once printed, but its not considered complete until I answer questions about who diagnosed me and what dates i was diagnosed. What should I do? Should I just type that it was a mistake on the online follow-up questions?

i messed up and said yes for something too. i just answered all the questions by putting 01/1900 (for dates) and in all of the options to put text i put "mistake made on form". i then printed it out and put a large ink line through it and initialed and dated it. i included it in with my packet, and was still cleared even though i messed up. make sure that on the front form where it asks all of the questions that you change that answer to "no" and initial it
