DODMERB medical history question

In completing the medical history for the exam is it appropriate to put a cover letter on the package that you bring to the exam. The cover letter would address some of the medical history that might cause a remedial or DQ and is designed to demonstrate that there are no impacts from the history and that superior physical and athletic results are now the norm with no real risk factors. Is the cover letter a good idea in general? Thanks!

Form DD2493 - DOD MEDICAL EXAMINATION REVIEW BOARD (DODMERB)
REPORT OF MEDICAL HISTORY - has a section called "REMARKS" where you can add info, explain a "yes" or "no" answer in more detail.

I would use that space, and attach a separate sheet if additional space is necessary.

Agree with David. If you are utilizing DODMETS, the system will automatically attach an additional page or pages if needed. Hold onto those records for if/when DoDMERB requests them.
