Received the following remedial codes today:

R259.10
R230.00

R259.10: We have determined that you do not meet the DoD medical standards for the following reason(s): $$$. Your file will require a review by the medical Waiver Authority of the Service Academy(ies), ROTC program(s), or other program(s) to which you have applied. However, before the waiver process can be initiated, you must complete the AMI(s) listed below: After we receive and review your AMI(s), we will send your entire file to the Waiver Authority for medical waiver consideration, based on their individual processes. Additionally, after receipt of the requested AMI(s), we will send you a letter that will explain the waiver process for each program. PLEASE REVIEW THE REQUESTED AMI(s) BELOW CAREFULLY AS THEY MAY BE EITHER "MEDICAL", "ADMINISTRATIVE", OR BOTH AND FOLLOW THE SPECIFIC INSTRUCTIONS FOR COMPLETION.

R230.00: DOES NOT REQUIRE A PHYSICIAN'S APPOINTMENT: Please provide copies (NOT ORIGINALS) of ALL mental health/medical treatment and/or hospitalization records REGARDING $ $$ , to include ALL applicable inpatient and outpatient records including, but not limited to, VA disability/examination records, DD Form 214, operative reports, anesthesia records, pathology reports, follow-up visits, rehabilitation records, recovery room records, x-ray reports, etc.

But the short description for R230.00 says: "Provide copies of all mental health records regarding $"

Does that mean I just send records regarding my mental health or just my full medical record? I'm confused because the short description seems to contradict the long description
 
There's no contradiction.

Short: R230.00 says: "Provide copies of all mental health records regarding $"

More expansive R230: Describes what would be included in the short version :)

My DQ code came in as:
R259.10Applicant pending Disqualification for D241.30 - Asthma/reactive airway disease/exercise induced bronchospasm after age 13 , must complete AMI(s) before waiver processing may be considered

So, I had asthma up until I was about 12... is there any way I can send in medical records (there's a letter from an allergist and records showing I didn't visit the doctor for asthma after 12 and like 3/12 months or something)? Or is it too late?

Related question, thought I would post here. Researched here and in "questions about the process" but still unclear.
Physician's Involvement vs. Physician's Appointment:

I think this question is generic enough that it doesn't require a FAST email. The "Required "administrative" AMI" indicates "does not require a physician's appointment." So....there are NO formal evaluations, assessments or reports regarding the issue in question. However, maybe the physicians have some of their own, personal notes on file. Are we expected to try and obtain these? Wasn't completely clear since it has the statement "appointment not necessary" does that mean, give us what you have/it should not necessitate the physician's involvement? Or do we need to check to see if the physician has any notes on file?

Sincere question here...just want to make sure we're answering it completely and correctly. Thank you in advance.

Edited to add, this is not a waiver review, rather "AMI is required before we can make a final determination."
