DoDMERB Remedial Requested

I recently received my examination from DoDMERB and received a remedial request (codes R251.15 and R251.12). Where do I send the completed the forms? It says to send them to an address at the Air Force Academy, but I'm only applying to USMA and USNA. Is that just where the review board is located? Any clarification is useful, thanks.
 
Yes the review board is USAF. There should be an email address in your letter you can send a scan of materials to.

DODMERB serves ALL branches but is headquartered at the Air Force Academy.
