I’m also curious the difference (if any) between DODMRRB DQ’s and ANG DQ’s. I have a son who is currently awaiting an ANG waiver. He never applied to a SA, so no DODMERB. Only MEPS.
DODMERB is primarily focused on pre-comm officer accession programs:
“ABOUT DODMERB
DoDMERB (The Department of Defense Medical Examination Review Board) is the Department of Defense Agency responsible for the determination of medical qualification of applicants for appointment to a United States Service Academy, the Uniformed Services University of the Health Sciences, the Reserve Officer Training Corps (ROTC) Programs of the United States Armed Forces, and other programs as assigned by the Assistant Secretary of Defense for Health Affairs.
The DoDMERB goal is to accomplish the mission with the highest degree of accuracy, at the minimum cost for the government and the applicant, in the least amount of time.
The DoDMERB staff is comprised of approximately 21 military and 23 civilians. The military is represented by members of the Army, Navy, Air Force, and Coast Guard. The Director value is rotated among the Services.”
MEPS is primarily for enlisted paths:

I believe (operative word) the accession standards are the same for all components, with some DQ standards different between officer and enlisted, due to job requirements. The waiver policies are what will vary widely.
It’s not clear to me where people going straight into the Guard, not from active or Reserve status, do their medical screenings, and whether it differs for officer and enlisted programs. That may vary by state.
The waiver policy is the pivot point.
