It's just some statements, and they rank how they feel about you for each statement on a scale of like 1-5.

I used to have a copy of what they were being asked but I couldn't find it. From what I recall it was 4 to 5 questions about their thoughts on
your leadership capabilities, how dependable you were, were you a take-charge type person, etc. I do recall thinking to myself that a number of the attributes they asking about were not something that every teacher would have the opportunity to observe in a classroom setting. That may be why they specifically want the gym teacher's input.
