So I received a copy of my medical records and on it, it has Chronic otitis media and acute otitis external. Are these waiverable? What documentation do I have to provide?
Same thing, it has migraine headaches on my history. These were back in highschool, which I graduated high school 3 years ago and have had no head ache issues. What documentation do I need for this?
