eczema question

Good morning,I am filling out the dodmerb application, and in the question that asks about eczema I inadvertently answered yes. I have never been diagnosed with eczema but I tend to get dry spots on my wrists and back of my knees that I take care of by applying over the counter cream. How do I go about removing the "yes" or is there a place for me to explain my answer? Also I have a question about the Have you ever had any medical treatment/surgery from a hospital, emergency room, etc. I never had surgery but I did break my finger on my right and on my left hand (two different times while I was goalkeeping) should I answer yes to this?
 
Good afternoon, I am looking for Mr. Mullen. I attempted to email according to the instructions that I have seen throughout the threads, but I"m not sure if I have the correct email (s) or if in fact if they went through (I have emailed through my school account). Just want to make sure I'm doing the right thing. Thank you!
